I actually found deep heat more tolerable than tooth paste personally. I think toothpaste is more abrasive to the actual flesh of the rectum, even if the deep heat hurts more while its happening.

Though I would play with vapo rub first and see if you can tolerate that as it is a milder version of the same sensation.
