When your month of no orgasms is over your first orgasm after it must be during sex and it must be with the permission of your sex partner and only after they've orgasmed. As in, explicitly the rule and explicitly asking if you are allowed to cum. And once you've asked someone, only they are allowed to give permission.

The fact that each time they deny you, they get laid again is deliberately perverse incentives to keep you denied and needy.
