I will add to this form as needed, but I will elaborate on it. Name, not username. Not necessarily your real name but something to call you. Sex, not gender, either male or female. Likes will probably not be used much, as this is a punishment. Dislikes are the most likely. Soft Limits, limits that you may be willing to break under certain conditions, may happen as well. Hard Limits I understand, non negotiable. I realize that this may be in your signature, but it may change. Also, remember that anything not in your hard limits written ON THIS FORM may be used. Don't just write, "see signature. " I won't put anything illegal or extremely obvious limits, but less known limits, such as a public limit, even hidden public, I should to know. If you miss a limit, we will try and figure something out. But please, don't make.is go through the hassle.

What you did to deserve the punishment MUST be detailed. Remember that the best written answers will receive punishments first. Punishment severity, from 1-10, extremely light to complete insanity. I suggest you start around the four, five, six range. Mental degradation, name calling, insults, etc., is completely optional, as I know it's not everybody's thing. For aftercare, if you write an amazing report, more about that later, you could receive high praise, comfort, and rewards from Mistress if you answer yes. Living Situation is basically noise level. If you have a roomate or live with family, it will need to be quieter than an apartment. Which should be quieter than a house by yourself. Now, my expectations.

You must send in this form, completely filled out. You may receive some leniency, but the ones with the best forms will get punishments sooner, and more detailed. I put a lot of thought into my punishments, so you must be worthy. After you perform your punishment, you WILL write me a detailed report on it. If I do not receive this report, in detail, you will not get anymore punishments.

However, I will hold myself to the high standard I hold you. If I do not give you a punishment within one week of the request, you can send me a harsh punishment, don t hold back. I will complete this punishment and send a complete report back to you, along with your requested punishment.

Post your completed form below, and I will PM you. If I don't respond after about 3 days or so, try sending me a PM. Those usually show as a notification, not sure if thread replies do.

i doubted my Holder, i thought She didn't care. Also, i doubted Her sincerity and behaved like a spoilt child about it.
