I have a couple but my worst one is trypophobia - fear of holes.
Just thinking about it makes me break out in goosebumps.

I have terrible arachnophobia, whichever one the needles one was, and one of my worst is blindness. I get vertigo, which i dislike, but I'm not actually afraid of heights. More afraid of falling from heights which could cause injury, but I suppose that's common sense?
