So i'm unsure for the game itself as I never played or heard of it. BUT Start a new game and set any difficulty to max. Play as your least favorite method or route and each time you die, You have to keep count and crush your testicles for that many minutes. After you beat the game or after 6 hours or so, which ever you want, You have to edge yourself that many times you died.
