do an enema at least half of a 2 liters worth and hold it in find a medium sized object to stick up your anal hole and use it to plug yourself every 5 mins take out the object with out letting your enema go and insert something larger do this till you have the biggest thing you can find in and leave it till you cant hold the enema anylonger

For my money, it should be at the start of pain and kept that way until there is no pain. At that point, after a period of a few days, it should be ramped up.

That is, if you're going for a gradual permanent gape. Otherwise going to "full powah" will result in less sustainability and hence less/no gape.
