The last one of these I saw was a little complex so here is one my wife and I made with the suggestions of Samishere.

Roll 1: time = 10 minutes X role

Roll 2: location/position
1 Indian style
2 Kneeling
3 standing
4 nose pressed in corner
5&6 caged


Roll 3: conditions
1 blind folded
2 gagged and bound
3 naked
4 collared and leashed and 3
5 both 1 and 2
6 all of the above

If the slave/sub was bad incorporate extra rolls.
Roll 4: extra time. Same formula as roll 1.

Roll 5: extra conditions.
1 all things from roll 3
2 butt plug
Roll for size:
1-2 small
3-4 Medium
5-6 Large
3 chastity device
4 chug 2 bottles of water. (No pee until released)
5 all from roll 5
6 all conditions roll 3 and 5
