To answer your question, generally the head is poking out the top, but not completely...the ring is still right around the glans. Part of this is because I'm not excessively large, so when I get soft after cumming and during post-orgasm-torture, I could slip out otherwise.

Well that didnt go entirely as I'd hoped for.....

So I got every thing ready, tied my anckles to the end of the bed, and used a belt around my thighs.
