Suggest doing this bottomless:
Wrap a string around your balls, and tie the other end to the brake pedal of the car in such a way balls get pulled when hitting pedal.

Keep drinking water (Safely) until you get the urge to urinate. Wait as long as you can without leaking any urine until you cannot bear the pressure on your bladder anymore. Then get into your car and get onto the highway without any exits for the next x miles ahead. Either torture yourself with the huge urge to urinate or relieve yourself in your car seat and keep on driving in your own smelly car.

And like MrCrazy said, only attempt this dare if you are confident that you can drive safely (with the urge to urinate).
