As already said, soak the q-tip with lube (I use saliva) so the cotton stays on the tip. But if you have doubts, read more on the topic of sounding and consider buying a couple of sounding rods to be safer.
In my experience, damaging the inside of the urethra is temporary. But make sure the items you insert are thoroughly cleaned and/or sterile. I consider q-tips clean enough if you are careful and don't damage your urethra. If you think you damaged your urethra then urinate.
