Extreme and smelly?
No problem.
When you have some spare time.
Let's go to the toilet and poop.
However the underwear will stay on to catch all poop.
Now remove underwear and place on head so you have a face full of poop to smell. Let's sit on the toilet for the next 10 minutes while smelling your treat.
Bonus if you open and close the mouth every minute, but anything that enters mouth has to be swallowed.
Once 10 minutes are up you can wash the underwear in the toilet.
Wring them out because you have to wear these for at least 24 hours
Now clean up your ass, preferably without paper.
