Very good question!
Most of the time I chose one role. If I'm playing with a dom, I sub all the time and if I'm playing with a sub I always dom.
Sometimes I play with switches or somebody that want to try to switch for the first time. I usually just agree on a role for the session.
It is very hard to switch during the session and most of the time it is more fun to play just one role. Sometimes I play with switches that are more into one of the sides ("switch more dom" for example). In that case I usually sub but sometimes I ask them if they want to sub for me. I always discuss the topic before the session and make it clear what are the expectations.
Open discussions and honest feedbacks before and after the sessions are the best way to have lot of fun
I switched in the middle of a session a couple of times but it really didn't work so well. Nowadays I tend to stick with a role for the session and switch in the next session or whenever we feel like to switch.
The determination of who is sub and who is dom is usually discussed before the play according to our desires of the day (and a little of compromise
