Not often, but I have before. A lot of shorts and stuff will have "built in" underwear as well.

Not really. Usually it's shorts/pants and a sports bra, sometimes have a shirt as well.

At one of my schools you weren't allowed to wear your ordinary pants under
your sports shorts, had to wear swimming trunks.
Nobody much bothered to do this and wore their usual pants.
Sometimes they would check and make you take your pants off and go
commando under the shorts.

Those were the times you would be extra careful about being pantsed!
