The thing is, he's a male. Always has been. Then we've had female Time Lords who were/are always female. They mentioned the Corsair who apparently changed gender but I like to think this was a one-off, or a joke. The can regenerate into any species but will always be male.

That said, his accent and skin colour... anything goes. I think the Doctor should always be British but an American accent wouldn't be too hard to cope with after having a Doctor with a Scottish accent in the past.
