Well, I hadn't thought about any penalty for breaking, because I don't expect you to break your rules
But in the very unlikely event that you unintentional break the rule, the penalty will be the same as for breaking your rule number 2 (counts as two orgasms).

will of course not happen but if it did, the consequence, besides that it counts as two orgasms, would be that you must delete that photo and any other nude photos you might have of the same woman and
never see her naked again. She will be lost for you forever, at least as uncensored naked. If you also have censored or clothed photos of her, you may however keep those.
